I am craniopharyngioma survivor with Sjorgen's syndrome. My mouth is so dry it is constantly painful. There are so many foods I simply cannot eat because it is so dry. I have severe gum disease and the teeth in my upper jaw have crumbled away to nothing - quite painlessly. This is post radiotherapy. Nobody is even vaguely interested, but I am choking on food. I already have hypothalamic anorexia and my diet is poor as it is - so I do really need help with what sounds like a trivial problem.
I don't think I have known a patient be referred for hypertensive retinopathy - diabetic retinopathy, yes. I think that they ought to be, but I have never actually known it to happen in General Practice.
This guideline is very clear and concise and very useful to have. Whilst I appreciate it should be used alongside the full NICE Guidelines, it is an effective and easy to read summary.

This has been an enlightening and very thought provoking module. It is not something I have had to do (yet) but I am sure that the day may come.
I think that (clinical expertise aside) it depends upon how it is presented to patients.
It is good to know that we can really contribute
I have downloaded the patient decision making aid. It looks really useful.
